Who is Eligible?

Chronic Disease Management services are available to Medicare card holders who have a chronic or complex medical condition that has been present (or is likely to be present) for six months or longer. This includes, but is not limited to, conditions such as diabetes, cardiovascular disease, arthritis, asthma, and other ongoing health conditions that require structured care.

Your GP will assess your specific healthcare needs to determine eligibility for a GP Management Plan (GPMP) and/or Team Care Arrangements (TCA). Under Medicare guidelines, eligible patients may access up to five Medicare-subsidised allied health services per calendar year as part of their care plan.

The length of your treatment will vary depending on your specific injury, your goals, and how you progress. Your Exercise Physiologist will work with you to develop a personalised plan, and the duration will be discussed and adjusted as needed.
